1. Description of Service: I understand and agree that permanent jewelry involves the process of welding jewelry together while over my body.

2. Risks of Welding: I understand that the welding process is a service that is considered a potentially dangerous activity and involves risks, including but not limited to:

• Potential shock: I understand that I may experience a minor, brief shock during the welding process.

• Burns: I understand that the welding process generates heat and could potentially cause minor burns to my skin.

• Discomfort: I understand that the welding process may cause discomfort due to the heat, noise, and vibration.

• Allergic reactions: I understand that I may experience an allergic reaction to the metal used in the jewelry or the welding process.

• Irritation: I understand that the welding process may cause localized irritation to my skin, or skin sensitivities.

• Physical or psychological injury: I understand that there is a risk of physical or psychological injury during the procedure, including but not limited to scarring, nerve damage, or emotional distress.

• Property damage: I understand that the welding process may damage clothing or other personal belongings.

• Disability: I understand that in rare cases, the welding process could result in temporary or permanent disability.

3. Assumption of Risk: I acknowledge that I have read and understood the risks associated with permanent jewelry and the welding process, and I voluntarily choose to participate in this service despite the risks involved. I assume all responsibility for any injury, damage, or loss that may occur to myself or my property as a result of my participation, including those potentially related to undisclosed medical conditions or pre-existing health concerns.

4. Permanency and Removal: I understand that the jewelry is intended to be permanent, but there may be instances where removal is necessary, such as for medical procedures like MRIs or surgery. I understand that the jewelry can be easily removed with scissors and re-attached by a jeweler at a later date, which may incur additional fees.

5. Durability: I understand that the jewelry, though durable, is not indestructible. It may be susceptible to damage from various factors, including but not limited to, snags, forceful pulls, or exposure to harsh chemicals or sulfur exposure. I agree to handle the jewelry with care and avoid any activities that could damage it and take full responsibility for such wear and/or damage to the jewelry. And, I agree to abide by the jewelry care instructions recommended by the jeweler. 

6. Jurisdiction, Venue and Governing Law:  Any and all disputes arising out of or relating to this Release and the permanent jewelry welding service shall be governed and construed in accordance with the laws of the State of Ohio], without regard to its conflict of law provisions. Further, any and all disputes shall be brought and heard exclusively in the courts of the State of Ohio, County of Delaware. 

7. Informed Consent: I understand that I have the right to refuse or discontinue the service at any time. I have had the opportunity to ask questions and receive answers to my satisfaction concerning the risks involved. I hereby freely and voluntarily waive any and all claims against the permanent jewelry provider, its employees, and agents for any injuries, damages, or losses arising out of my participation in this service. I give my informed consent to the permanent jewelry welding attachment service.

By signing below, I acknowledge that I have read and understood this Release and agree to be bound by its terms. I confirm that I am at least 18 years of age and legally competent to enter into this agreement. 

By signing below, I acknowledge that I am the minor’s parent or legal guardian and hereby consent for my child for the permanent jewelry service. I have read and understand the terms of this Release and Waiver of Liability and agree to be bound by its terms. I acknowledge and assume all responsibility for any injury, damage, or loss that may occur to my child as a result of their participation. 

GENERAL POLICIES

All sales are final, with no refunds for broken jewelry. 

We offer a repair policy that includes free re-welding, but the original chain/jewelry must be present. 

You must agree to wear safety goggles during the permanent jewelry welding process. 

Avoid regular direct contact with perfume, body lotions, hair sprays or other chemicals.

Changes in body chemistry can tarnish chains over time.

We are not responsible for misplaced or broken chains or reaction to metals.

MEDIA RELEASE

Please read carefully before signing:

I, the undersigned, hereby grant the permanent jewelry provider, Forever Link, LLC (the “Company”), the non-exclusive, royalty-free right and permission to photograph, film, and record my image, voice, and likeness (collectively, "Materials") in various media formats during the permanent jewelry service.

I understand that the Materials may be used for marketing and promotional materials, public relations and media campaigns, and educational purposes related to the Company. 

I acknowledge that I have no ownership rights to the Materials and that all intellectual property rights remain with the Company. I waive any right of inspection or approval of the finished product. I understand that I will not be compensated for the use of the Materials.

I understand that I have the right to revoke this permission at any time. However, I acknowledge that any use of the Materials prior to my revocation will not be affected.

I hereby release and forever discharge the Company and its assigns, licensees, and successors from any and all claims, demands, and liabilities arising out of or related to the use of the Materials.

I acknowledge that I have read and understood this Media Release and freely and voluntarily agree to its terms and that I am of legal age.
